203 - Decisional capacity and advance care planning in older people who are incarcerated

Abstract There is a growing number of older people incarcerated across the United States. With population greater than 300 million, US has 5% world’s population, yet incarcerates 25% prisoners. From 2000 to 2005, percentage prisoners in federal and state correctional institutions who were 55 increased by 33%. According American Civil Liberties Union prison climbed 1300% since 1980s, with 125,000 inmates aged or incarcerated. Correctional facilities are largest mental health 1 out 5 individuals serious substance use disorders. These lack capacity provide long-term care for those severe physical disorders even most ideal circumstances. Individuals within criminal justice system have higher burden chronic lower life expectancy. Health decision making one area where patients custody autonomy discussing advance directives, substitute makers medical making. However, at risk suboptimal care, unmet palliative end needs, lacking inappropriate surrogates. Without documentation directives surrogates there bureaucratic, practical, legal barriers particularly without family friends. In addition, some involved system’s only surviving members may also be their victims, thus conflict interest.
